Terminating Registration of Radiation Producing Machines

To terminate your Certificate of Registration the following requirements must be met in accordance with 25 Pa. Code § 216.4a:

You no longer possess any X-ray equipment.
Request termination of your registration by completing the form hyperlinked below.
Submit payment for any outstanding fees to DEP as per 25 Pa. Code § 218.11.
Inform the DEP of the disposition of all X-ray equipment by completing:
